Home Care Physical Therapist provides therapy services to patient's at home. The therapist skills includes:
Physiological Knowledge; Health Promotion and Maintenance; Creating a Safe, Effective Environment; Motivating Patients and Caregivers; Legal Compliance; Quality Focus; Bedside Manner; Clinical Skills; Pain Management; Persistence; Energy Level.
The therapist evaluates need for rehab services, establishes treatment plan in collaboration with the patient and physician, administers therapy as planned, including use of appropriate devices and exercise techniques; evaluates effectiveness of treatments; recommends referrals and other treatment plans; completes discharge planning in collaboration with the healthcare team and patient and caregivers.
PTA supervision, as needed. OASIS proficiency preferred but not required. Requires negative TB skin test, CPR Certification, Current MI PT License, Hepatitis B Vaccination, Valid MI Auto Insurance and Driver's License. Polite, Reliable and Dependable.
